Job Summary
Provide and coordinate care for assigned Home Health patients under RN supervision while developing nursing care plans and educating families on disease management. Assess patient status on an ongoing basis, document charting, and notify physicians of significant status changes. Ensure all orders are transcribed appropriately per state Nurse Practice Act regulations and facility policies. This full-time, travel contract position offers a guaranteed 40-hour week from 8:00 a.m. to 5:00 p.m., Monday through Friday, with a weekly gross pay of $1680 including travel and housing stipends. Candidates must hold an LPN license, one year of recent home health experience, and HCHB charting proficiency.
Required Qualifications
- 1 - 2 years LPN experience
- Recent/Current Home Health Nursing experience: 1 year within the last 3 years
- Home Care Home Base (HCHB) charting experience
- LPN License for the state applying or Compact if accepted
