Provide and coordinate care for assigned hospice patients in the comfort of their homes, developing nursing care plans including Start of Care and admissions. Support patient recovery by educating families on disease management and ensuring physicians' orders are transcribed appropriately. Assess patient status on an ongoing basis, document all visits within 24 hours, and notify physicians of significant status changes.
