Job Summary
Provide Physical Therapy services to patients at their homes in Marshfield, WI and surrounding areas. Consult with physicians and interdisciplinary teams to develop treatment plans, utilizing evaluation skills to restore function, relieve pain, and prevent disability. Drive daily to patient homes within assigned territories, ensuring safe, effective rehabilitation while maintaining patient dignity. Work 40 hours per week, Monday through Friday, 8 a.m. to 5 p.m., with no weekend or call rotation. Requires current Physical Therapist license, 12 months of recent Home Health experience, and valid driver's license.
Required Qualifications
- Current Physical Therapist experience: 1-2 years
- Recent/Current Home Health PT experience: 12 months within the last 3 years
- Home Care Home Base (HCHB) charting experience
- Valid Driver's License
- Personal Auto Insurance
- Covid Vaccination or medical or religious exemption
- Bachelor of Science degree in Physical Therapy or Similar
- Current State License as Physical Therapist or Compact if applicable
- BLS Certification from AHA or American Red Cross
