Job Summary
High Risk Clinical RN provides direct home-based clinical care for high-risk patients, including wound care and wound vacs, IV antibiotics, and comprehensive safety checks. Documents findings in EMR the same day, communicates with Wound Care Specialist, PCP, and care team, and coordinates timely follow-ups and discharge planning to prevent readmission. Requires strong organizational skills, ability to work under pressure, and adherence to patient confidentiality. Must be a state-licensed practical nurse with a practical nursing program background and LVN experience (minimum 3 years); clinic and managed care experience preferred; proficient in ICD-9 terminology and computer use.
Required Qualifications
- Graduation from an approved practical nursing program
- Must be state-licensed practical nurse
- minimum three years LVN experience
- clinic experience preferred
- Prior managed care experience preferred
- knowledge of medical terminology
- understanding of ICD-9 coding
- excellent customer service skills
