High Risk Cardiac Clinical Nurse

Assess, diagnose, plan, implement, and evaluate patient care within high-risk cardiac guidelines. Educate patients and families on illness management, home care needs, and medication administration. Participate in activities promoting patient safety, quality, and regulatory compliance. Develop and document individual plans of care with defined goals in collaboration with the interprofessional team. Proactively communicate care plans across the continuum while assessing physical, psychosocial, and emotional needs. This safety-sensitive role requires constant alertness.
