Job Summary
High Fidelity Wraparound Facilitator tasked with providing on-going clinical management of wraparound referrals/cases, maintaining credentialing, coordinating care with staff and external agencies, ensuring compliance with billing/documentation, facilitating data-driven quality services, and leading team meetings and stakeholder education, while pursuing required certifications and ongoing professional development.
Required Qualifications
- Bachelor’s degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree with two or more years of clinical experience with youth
- Qualified as an OBHP per 405 IAC 5-21.5-1(d)
- Indiana certification as a Wrap Facilitator preferred
- Minimum of five years of experience with Seriously Emotionally Disturbed (SED) children and families preferred
- Prior Wrap Facilitation experience preferred
- DMHA and OMPP-approved training and certifications for CMHW services required
- Eligibility for certification as a CANS SuperUser
- Wraparound Practitioner Certification in accordance with DMHA policy in process or obtained
- Strong leadership, communication, and organizational skills
- Valid driver’s license
- Must maintain membership in Southwestern Clinical Staff Organization (CSO)
- NAPPI certification must be obtained and maintained
- Background checks including county/state checks, fingerprinting, CPS check, sex offender registry check and drug screen required
