Job Summary
Care Partner provides one-on-one in-home dialysis support for a patient transitioning from clinic to home. Responsibilities include setting up dialysis treatments (NxStage or Tablo), keeping the treatment environment clean and safe, offering companionship, helping with scheduling, providing light ADL support, monitoring safety, reporting concerns to the coordinating nurse, and maintaining simple documentation/shift notes. Training will be provided by the clinical team. Requires dialysis experience and ability to work with a clinical team; must pass a background check. Location: Chicago, IL; schedule 4-5 days per week; patient-focused role with independent work alongside a dedicated care network.
Required Qualifications
- 1+ year of dialysis experience (home or clinic)
- Familiarity with home dialysis systems preferred (NxStage, Tablo is a plus)
- Open to in-home dialysis support and willing to complete the provided training
- Strong communicator who works well with a clinical team
- Must pass a Herewith background check
