In-home dialysis care partner for a 1:1 client in Chicago, providing setup assistance for NxStage or Tablo dialysis, maintaining a safe and organized treatment environment, offering companionship and light ADL support, ensuring adherence to treatment schedules, monitoring safety, and documenting shift notes while coordinating with a clinical team during the patient’s transition to home-based dialysis.
Required Qualifications
- 1+ year of dialysis experience (home or clinic)
- Familiarity with home dialysis systems preferred (NxStage, Tablo is a plus)
- Open to in-home dialysis support and willing to complete the provided training
- Must pass a background check
