Job Summary
Provide in-home patient-centered care as a Home Health Aide (HHA) / Certified Nursing Assistant (CNA), assisting clients with activities of daily living (ADLs) such as bathing, grooming, dressing, toileting, mobility and transfers, and offering companionship. Follow established care plans under the direction of a Registered Nurse, observe and report changes, assist with medications reminders, and maintain HIPAA-compliant documentation. Travel to client homes within the service area (up to 45–60 minutes between shifts) and collaborate with the care team to ensure safe, dignified, and high-quality care for clients.
Required Qualifications
- Current Home Health Aide (HHA) certification (or CNA if applicable per state requirements)
- Valid driver's license
- Reliable transportation
- Current auto insurance
- Willingness and ability to travel within service area
