Provides direct-care support in a residential program as a Home Health Care Aide/CNA. Responsibilities include promoting health, assisting with mobility and transfers (may use mechanical lift devices), meal preparation, housekeeping, incontinence care, transportation needs, and participation in regular staff meetings. Requires communication with individuals and team, confidentiality, and adherence to safety and funding regulations. Requires a CNA/HHA role with awake overnight coverage on the Thu–Sat 9pm–7am shift; must have a High School diploma or GED, and CPR/First Aid certifications plus a valid driver’s license. Ability to read and interpret safety materials, write routine reports, and operate with professionalism in a team environment is expected.
