Job Summary
Conduct daily personal care including bathing, oral care, cooking meals, and grocery shopping. Assist patients with daily living activities per their Plan of Care, track their condition, and report unusual changes to the agency. Accompany patients to doctor's appointments, handle basic housekeeping, and manage essential errands. Communicate any issues with Summit Home Care while following agency policies. Maintain required certifications, complete orientation for a $50 bonus, and adhere to full-time or part-time schedules with flexible hours.
Required Qualifications
- New York Home Health Aide Certificate or Personal Care Assistant Certificate
- Government issued ID
- Social Security Card
- Work permit, if needed
- 2 Reference letters
- Recent physical
- Quantiferon
- Rubella and Rubeola lab work
- Chain of Custody Drug Screen, no older than 6 months
