Job Summary
Provide routine daily nursing care and services to assigned residents in accordance with their individual service plans. Document all care provided in the EMR and report changes in resident condition to the LPN. Assist with grooming, bathing, dressing, and incontinence care while ensuring proper body mechanics during lifting and moving. Transport residents to appointments, activities, and dining rooms, and prepare residents for meals and assist with feeding. Answer resident calls promptly, maintain confidentiality of protected health information, and report any incidents of abuse or unauthorized access. Participate in scheduled training, orientation, and in-service programs to maintain certification. Work throughout the nursing system area, including weekends and holidays, while adhering to safety precautions and isolation protocols.
Required Qualifications
- High School Diploma or HSE
- An active and valid HHA certification
- Proficiency with computers and standard office software
- Must be able to read, write, speak, and understand the English language
- Demonstrated knowledge and skills necessary to provide care primarily to the geriatric population
- Candidates must have a start date on or before 9/30/2026
Desired Qualifications
- current Nursing Assistant certification in New York State
