Job Summary
The Home Health Nurse, RN administers skilled nursing care to patients requiring intermittent professional nursing services in a home health setting. Responsibilities include following the physician's plan of treatment, observing and documenting patient condition and responses to interventions, developing and updating the patient individualized plan of care, educating patients and families, coordinating with physicians and inter-disciplinary team, ensuring accurate documentation in the medical record and EMR, and supervising LPNs and CNAs to meet agency productivity standards. Positions require on-call duties, travel to assigned offices or patient homes in Clinton and Fayetteville, NC, and adherence to CMS/Hospice guidelines and agency policies. The role emphasizes compassionate, one-on-one patient care and collaboration with supervisors to support positive patient outcomes and compliant care delivery.
Required Qualifications
- Registered Nurse license to practice nursing in the state of North Carolina
- Minimum of one-year experience as a professional nurse in an acute care setting
- Maintain current CPR certification (BCLS)
- Basic computer skills/knowledge and experience in electronic medical records
- Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public
- Possess valid Driver's License and reliable transportation
