Job Summary
Administer skilled nursing care to patients requiring intermittent professional service, conducting complete assessments including medical history and physical exams to identify appropriate care needs. Develop and implement individualized patient care plans in the EMR while ensuring ongoing evaluation of patient/family needs and response to care. Document services rendered, conferences, and changes in condition electronically, including OASIS certifications and daily downloads. Provide regular supervision to LPNs and CNAs, ensuring tasks align with regulatory guidelines and CMS/Hospice standards. Collaborate with physicians to report reactions to treatments and alter plans of treatment as necessary. Manage assigned cases to achieve productivity goals while maintaining compliance with agency policies and state nursing laws.
Required Qualifications
- Registered Nurse currently licensed to practice nursing in the state of North Carolina
- Minimum of one-year experience as a professional nurse in an acute care setting
- Maintain current CPR certification (BCLS)
- Basic computer skills/knowledge and experience in electronic medical records
- Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public
- Must be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient's individualized a plan of care
- Possess valid Driver's License and reliable transportation
