Job Summary
Assure spiritual assessment of patients and families/caregivers in the hospice program and facilitate the development of individualized plans of care by participating in comprehensive assessments. Provide direct spiritual care, serve as a liaison to community chaplains and spiritual counselors, and maintain records of services for quality assurance and policy review. Attend patient care conferences and interdisciplinary meetings to provide consultation, education, and support on spiritual care needs. Recruit community chaplains, spiritual counselors, and volunteers by developing community contacts and offering education through congregations. Actively participate in quality assessment and performance improvement teams, and provide funeral or memorial services as requested. Assist in supervision of spiritual care volunteers when assigned.
Required Qualifications
- Graduate of accredited seminary or school of theology (BD, Mdiv or equivalent theological degree)
- Experience working with death and dying, individuals/family/caregiver
- Ability to work as member of interdisciplinary team/group
- Comfort in an interfaith setting
- Ability to accept different lifestyles, cultures, beliefs, and values
- Ability to network with community clergy and congregations
- Knowledge of and commitment to hospice philosophy of care
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
