Job Summary
Conduct case management for Healthy Start clients by performing home visits, initial assessments, and tracking procedures to manage appointments and referrals. Assist clients in accessing community services while providing education on smoking cessation, childbirth, breastfeeding, and parenting. Maintain client files and the Healthy Start database, completing documentation and coding for services within 72 business hours. Prepare and facilitate group connections, conduct client satisfaction surveys, and assist with monthly and quarterly reports. Provide office coverage and handle phone calls or visitors during required hours. Travel up to 75% of the time to visit settings such as client homes, courts, or other agencies.
Required Qualifications
- Bilingual in Spanish and English
- A bachelor's degree in Social Sciences, Social Work, Human Services, or a Health-Related Field such as Health Education, Child Maternal Health, or Public Health
- Valid Florida driver's license
- Must successfully clear Level II Background Screen
- Must possess an acceptable driving record
- Must have reliable transportation
- Excellent knowledge of community resources
- Excellent interpersonal and communication skills (verbal and written)
- Excellent computer skills in Microsoft Office Suite (Word, Excel, PowerPoint)
- Effective public speaking skills
- Ability to adapt to a hybrid working environment as needed
- Ability to travel up to 75%
- Ability to lift 10-25 pounds
- Ability to safely operate a motor vehicle
- Ability to work evenings and weekends as needed
- Ability to work in an environment with frequent interruptions and a moderate noise level
- Ability to independently complete job duties within required time frames
- Knowledge of family and systems theory
Desired Qualifications
- 2 years of related experience
- Experience conducting home visits with a similar population
- Knowledge of obstetrics, pediatric health, and health care systems
