Job Summary
Conduct daily rehabilitation support for up to 54 clients, fostering their independence across daily living activities through motivation and hands-on guidance. Build trust with clients while collaborating closely with nurses, caregivers, and other disciplines to facilitate their return to home or appropriate care settings. Work flexible shifts including days, evenings, weekends, and holidays, with options for both fixed and self-scheduled part-time roles. On-call availability for Somatiek, PG, or Zorg Thuis may apply. A dedicated buddy supports you during your first month.
Required Qualifications
- Helpende Zorg & Welzijn diploma niveau 2
- certificaat Helpende Plus of Medicatie
- bereidheid om het certificaat Helpende Plus of Medicatie via de Silverein Academy te behalen
- affiniteit met revalidatiezorg
