Heart Failure and Pulmonary Hypertension Nurse Practitioner

Nurse Practitioner sought to join Hackensack Meridian Health’s Heart Failure and Pulmonary Hypertension team at Hackensack University Medical Center. Lead inpatient care with a caseload of 5-10 inpatients daily, conduct new patient evaluations, and manage ongoing outpatient care. Utilize advanced clinical skills to interpret complex data, including hemodynamic monitoring from right heart catheterizations and Swan-Ganz catheters, to guide treatments for pulmonary hypertension. Collaborate with a multidisciplinary team to manage advanced medical therapies (IV inotropes and prostacyclins) and ensure seamless discharge planning. Provide comprehensive coverage across inpatient and outpatient services, contributing to high-quality patient care.
