Heart Failure and Pulmonary Hypertension Nurse Practitioner

Lead inpatient and outpatient care for Heart Failure and Pulmonary Hypertension patients. Manage a caseload of 5-10 inpatients daily, conduct new patient evaluations in the outpatient clinic, interpret complex clinical data including hemodynamic monitoring from right heart catheterizations and Swan-Ganz catheters to guide advanced therapies, collaborate with a multidisciplinary team to manage IV inotropes and prostacyclins, and ensure seamless discharge planning to provide continuity of care.
