Job Summary
Manage patient navigation, provide family education, and coordinate multidisciplinary care plans for complex cardiac conditions across inpatient, procedural, post-discharge, and outpatient settings. Serve as the primary liaison between patients, families, and healthcare providers to ensure seamless care coordination. Support quality and regulatory initiatives while monitoring program outcomes and promoting evidence-based cardiovascular practices. Collaborate with cardiologists, surgeons, and advanced practice providers to improve outcomes and enhance access to care. This role supports the multidisciplinary team at Valley Children's Willson Heart Center, which delivers top-tier pediatric cardiac care for congenital and acquired cardiovascular conditions.
Required Qualifications
- Bachelors Degree (preferred)erably in Nursing or Health Care Related Field (required)
- Or Equivalent (required)
- RN - Registered Nurse License - CA-BRN - California Board of Registered Nursing (required)
- BLS - Basic Life Support - AHA - American Heart Association (required)
- Minimum three (3) years Clinical, leadership and/or project management experience. (required)
- Must have excellent communication skills and the ability to collaborate effectively across diverse teams to achieve high quality outcomes
- Strong analytical and problem-solving skills required
- Must have leadership and project management abilities and extensive advanced knowledge of nursing or respiratory care principles, clinical practice, quality and healthcare systems
- Demonstrates proficiency in the skills and knowledge required to perform the job in specialty area
Desired Qualifications
- National Certification - Other National certification in area of specialty. (preferred)
