Job Summary
Conduct intensive home visitations to provide prenatal education, newborn care guidance, and breastfeeding support to pregnant women, infants, and children up to age three. Screen and assess referred families to develop individualized plans of care that address specific goals for birth and health outcomes. Facilitate access to community resources and parent classes while teaching positive parenting skills and smoking cessation strategies. Maintain accurate client case records and enter data into software systems in compliance with contractual requirements. Make home visits and travel to local business activities using a personal vehicle, while collaborating with supervisors to evaluate client status and participate in departmental training.
Required Qualifications
- High School diploma or equivalent
- Florida Driver's License within 30 days from hire with daily access to a reliable and insured vehicle
Desired Qualifications
- Bachelor's Degree in Social Work, Health Education or comparable human services field
- Licensed Registered Nurse
- Previous experience in a social-services organization
- Two years-experience in a social services, health care, public health or related field
- Experience with direct client care
