Job Summary

Direct supervision of the HealthLink Team to ensure compliance with all standards, monitor clinical care quality, and manage caseloads and productivity metrics. Provide coverage as needed to meet client needs and targets while reviewing documentation for accuracy against funder and CARF guidelines. Lead hiring and retention efforts, fill vacancies within two weeks, and train staff on program policies and monthly goals. Ensure each Care Coordinator maintains an 85-client caseload with monthly face-to-face encounters and up-to-date care plans. Submit team productivity reports by the fifth of each month and prepare quarterly QA/QI reports. Respond to all flags, emails, and voicemails within two business days and complete chart documentation within the same timeframe. Maintain positive relationships with internal departments and outside agencies to maximize program growth. Requires a dependable vehicle with certified driver status and a valid license with F endorsement.

Required Qualifications

  • One-year experience in the social work field or a behavioral health setting
  • Must have at least a Bachelor's degree in a health-related field of counseling, psychology, social work, sociology, or a behavioral science field
  • Course work or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and mental retardation
  • Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, or education must have fifteen college-level semester hours of coursework in behavioral health
  • Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, or education must have at least one year of work experience in the behavioral health setting
  • Valid driver's license- F endorsement
  • Must be certified in, and effectively implement, verbal and physical de-escalation techniques
  • Must be able to lift up to 50 lbs
