Job Summary
Healthcare Navigator role at Oakview Health & Rehab requires coordinating pre-admission and admission processes, maintaining frequent contact with patients/families, orienting new admissions, participating in the center’s interdisciplinary team, and developing partnerships with healthcare providers to ensure appropriate care settings. Responsibilities include managing admission paperwork, ensuring patient rooms are ready, guiding transitions, conducting outreach and admissions planning for nights/weekends, and promoting the organization’s service standards. Key skills include knowledge of Medicare/Medicaid/private pay reimbursement, strong communication and collaboration with departments and partners, proficiency with MS Office, and the ability to interpret safety and procedural documents within a long-term care environment.
Required Qualifications
- Bachelor’s degree in Business Administration, Social Work, Marketing or related field
- Associate’s degree in related field with three years experience
- Knowledge of Medicare, Medicaid, Private Pay and Third Party reimbursement
- Proficiency with Microsoft Outlook, Microsoft Excel, Microsoft Word
- Ability to read and interpret documents; communicate effectively; adapt to the center environment
- Understanding of long-term care admission requirements
- Certificate, licenses, registrations as applicable
