Job Summary
Healthcare Navigator (SSVF) supports homeless veteran families by connecting Veterans to VA health care benefits or community health care services, providing case management and care coordination, health education, and acting as liaison to VA/community resources. Works with an interdisciplinary treatment team, collaborates with Veterans and families, documents progress, and links Veterans to supportive services while traveling to various communities as needed. Requires related education/experience and a valid driver's license; operates within a residential facility environment and utilizes Microsoft Office tools.
Required Qualifications
- Associate's Degree with a minimum of two years of case management experience or closely related experience
- High School diploma or equivalent and a minimum of four years of case management experience or closely related field experience
- Valid driver's license
- Adept with computer programs, including but not limited to Microsoft Office suite and data information management systems
