Job Summary
Deliver compassionate, high-quality end-of-life care during day and night shifts in the Powys community, supporting patients and loved ones with personal care, symptom management, and emotional needs. Work under the guidance of registered professionals to contribute to care planning, assist with daily living activities, and maintain a safe, dignified environment. Prioritize and respond to changes in patient condition while collaborating within a multidisciplinary team. Full driving license and vehicle access required.
Required Qualifications
- NVQ Level 2 in Health and Social Care or equivalent Level 2 qualification (e.g. NCFE, BTEC)
- functional skills in numeracy and literacy at Level 2
- At least 6 months of recent care delivery experience in a health or social care setting
- Full Driving license and access to a vehicle
Desired Qualifications
- Knowledge of palliative and end of life care, including holistic care planning and symptom management
- Ability to plan and prioritise care, respond to changes in patient condition, and escalate appropriately
- Strong communication skills with the ability to hold person-centred, empathetic conversations and work effectively within a multidisciplinary team
