Job Summary
Health Worker I (Spanish-speaking) supports Medi-Cal members in outpatient substance use treatment and behavioral health by providing preventive health education, navigation services, and access to medical, dental, behavioral health, housing, transportation, and food assistance. Performs culturally responsive outreach to underserved communities, collaborates with multidisciplinary teams, documents care plans and progress notes, and may work off-site or after-hours for crisis or events. Requires Spanish-language proficiency preferred, First Aid and CPR within 60 days, and a valid California driver’s license. The role involves coordinating care, referrals, and client-centered support across services to promote health and recovery goals.
Required Qualifications
- High School Diploma/GED
- Experience in outreach, case management, or similar role in health or social services
- First Aid Certification within 60 days of employment
- CPR Certification within 60 days of employment
- Valid California driver’s license REQUIRED
- Background check and livescan
- Be responsive and proactive in correspondence with referring parties, judges, probation and parole officers, social workers, and the Quality Improvement Dept. via treatment status reports, etc.
