Job Summary

Conduct therapeutic rehabilitation activities, including planning recreational, educational, and work therapy sessions, while interviewing and screening patients to assess conditions and develop treatment plans. Assist in crisis intervention, coordinate care with professional staff, and represent the program in meetings with community groups and agencies. Supervise lower-level health workers, maintain patient charts, and implement social, creative, and religious programs as part of specialized activities. Work onsite across hospitals, clinics, and homeless support services, potentially in jails or psychiatric facilities. Requires two years of recent experience with diverse populations, bilingual proficiency, and specific certifications. Fulltime employment involves days, evenings, weekends, and holidays.

Required Qualifications

  • Two (2) years of verifiable experience within the last five (5) years, working with a culturally diverse population performing a combination of at least two (2) of the following duties: serving as a liaison between targeted communities and healthcare agencies; providing culturally appropriate health education/information and outreach to targeted populations; providing referral and follow up services or otherwise coordinating care; providing informal counseling, social support and advocacy to ...
  • Possession of a valid Alcohol and other Drugs Counseling Certification accredited by the National Commission for Certifying Agencies
  • Bilingual Proficiency in English and a target language
