Coordinate virtual visits, onboard patients to digital platforms, monitor remote exercise and symptom data, and ensure accurate documentation in the EHR. Collaborate closely with PT, OT, and SLP teams to streamline workflows, improve access, and enhance outcomes. Support delivery of outpatient rehab through telehealth and connected care tools.
