Job Summary
Obtain patient records from outside providers, follow up on referrals and diagnostic orders to completion, and provide care coordination services to at-risk patients. Perform outreach to patients due for care, deliver patient education on appointments and remote monitoring, and refer individuals to internal and external resources. Monitor phone calls and written communication to respond to inquiries, then track and document activity outcomes in logs with monthly statistics. Maintain confidentiality while interacting professionally with AHS staff and external stakeholders in a fast-paced, on-site environment.
Required Qualifications
- High School Diploma or equivalent
- Must have excellent customer service skills
- Must have knowledge of basic computer usage, including familiarity with the Microsoft Office Suite
- Must have the ability to approach and interact with patients, including interviewing patients and engaging them in appropriate services
- Must have the ability to track activities completed and services provided
- Must be able to maintain confidentiality
- Must have the ability to interact professionally with AHS staff
- Must have the ability to work in a fast-paced, patient care environment
- Must have the ability to travel between city locations
Desired Qualifications
- Some college or bachelor's degree
- Prior experience in providing direct services in a healthcare or human/social services organization
- Prior experience with electronic health records
- Knowledge of case management procedures
- Prior experience with external stakeholders
