Health Services Coordinator RN

Educate clients, families, and staff on health, wellbeing, and disease prevention while attending residents' meetings to provide health updates. Communicate with physicians regarding client needs, nursing assessments, and recommendations, and collaborate with team members to ensure well-being. Perform assessments and evaluations for potential clients, create nursing care plans, and implement individual care plans in accordance with established procedures. Provide basic patient care such as wound dressing, bed sore treatment, and medication administration while measuring and recording vital signs. Complete audits per regulatory requirements, ensure physical and immunization needs are met, and implement necessary diet changes.
