Health Services Case Manager

Job Summary
Manage a caseload of clients with high-service utilization and disabilities, including substance use and significant behavioral health needs. Provide patient-centered case management services focused on housing, public benefits, and medical/mental health referrals to end homelessness. Transport clients to appointments and conduct outreach to assess safety and resource needs. Respond to crises, de-escalate situations, and develop tailored safety plans. Mediate conflicts and maintain positive relationships with community partners and health providers. Track client data, engagement, and outcomes in the EHR while ensuring accurate charting. Assist clients in securing benefits such as Social Security and Medicaid. Work collaboratively within the integrated healthcare team under direct supervision to support collective problem-solving and continuous quality assurance.
Required Qualifications
- High School Diploma or GED
- Basic Life Support Certification
- Colorado driver's license
- 1-2 years of experience working with a diverse population experiencing homelessness, substance use disorders or mental health challenges, particularly those individuals diagnosed with serious and persistent mental illness (SPMI)
Desired Qualifications
- Bachelor's degree in Psychology, Social Work, Human Services, Criminal Justice
- Knowledge of local housing resources, benefits programs, and social service agencies
