Health Promotion Advocate, Per Diem, All Shifts, Emergency Department

Job Summary
Conduct patient interviews and administer evidence-based screening tools to identify substance use, behavioral health, and social determinants of health needs within the Emergency Department. Integrate health promotion, brief interventions, and referral services into the patient care model for Project ASSERT, prioritizing assessments based on clinical indicators and physician referrals. Provide motivational support, patient education, and care navigation to promote engagement in treatment, while coordinating referrals to internal and external community-based providers. Maintain accurate, confidential documentation and resource information in accordance with organizational policies and regulatory standards.
Required Qualifications
- Valid driver's license
- Proof of current liability insurance
- Reliable vehicle
- Bachelor's degree
- Three (3) or more years of experience in one or more of the following: Community or population-based health screening, Primary care or ambulatory care settings with significant patient interaction, Substance use treatment, intake, or behavioral health environments
- Knowledge of substance use disorders
- Knowledge of behavioral health screening
- Knowledge of motivational interviewing
- Knowledge of harm reduction
- Strong interpersonal and interviewing skills
- Ability to communicate clearly, professionally, and empathetically
- Accurate data collection and documentation skills
- Proficiency with electronic documentation systems
- Cultural competence when working with diverse populations
- Per diem employees are expected to work a minimum of 32 hours per month
- Weekends: if applicable
