Job Summary
Execute the MDS-HC for service determination within the Elderly and Disabled Waiver Program while functioning as part of an interdisciplinary, person-centered team. Determine eligibility for the program using intermediate level of care criteria, meet standards for admission referrals and annual assessments, and explain program aspects to clients and representatives. Collaborate with Care Coordinators to develop Comprehensive Care Plans, act as a liaison between clients, families, and providers, and attend hearings as requested. Maintain knowledge of relevant manuals, report suspected abuse, and adhere to HIPAA guidelines. Requires a Georgia RN license, two years of experience in geriatric or community health nursing, and valid driver's license. Travel is involved across a 35-county region.
Required Qualifications
- Graduate of an accredited Nursing School or Program
- Two years' experience as a Registered Nurse in one of the following areas: Geriatric nursing, Community health, Long term care, Chronic diseases of adults
- Valid State Driver's License
- Current valid Georgia RN License
- Ability to communicate and coordinate effectively with clients, family members, service providers, other agency staff, and care coordination staff
- Proficient computer skills with ability to learn agency systems
- Well organized and detail oriented
- Demonstrates knowledge of health care and human services programs
- Knowledge of community organization and service system development
- Knowledge and understanding of the CCSP program
- Required travel within the 35-county region
- Required to inspect and safely operate a motor vehicle in a wide range of weather and traffic conditions
- May be required to walk on uneven terrain to get to client homes
- The employee must occasionally lift and /or move up to 25 pounds
