Job Summary
Execute the MDS-HC for service determinations within the Elderly and Disabled Waiver Program by assessing eligibility, conducting annual and admission referrals, and collaborating with Care Coordinators to develop Comprehensive Care Plans. Clarify client needs during case conferences, explain program aspects to representatives, and act as a liaison to resolve complaints or non-compliance issues. Report suspected abuse, attend hearings as requested, and maintain adherence to HIPAA and program manuals. Requires a current Georgia LPN license, two years of experience in geriatric or community health, and valid driver's license for travel across the 35-county region.
Required Qualifications
- Graduate of an accredited Nursing School or Program
- Two years' experience as a Licensed Practical Nurse in one of the following areas: Geriatric nursing, Community health, Long term care, Chronic diseases of adults
- Ability to communicate and coordinate effectively with clients, family members, service providers, other agency staff, and care coordination staff
- Proficient computer skills with ability to learn agency systems
- Well organized and detail oriented
- Demonstrates knowledge of health care and human services programs
- Knowledge of community organization and service system development
- Knowledge and understanding of the CCSP program
- Valid State Driver's License
- Current valid Georgia LPN License
- Required travel within the 35-county region
- Attendance at off-premises meetings, conferences or trainings
- Required to inspect and safely operate a motor vehicle in a wide range of weather and traffic conditions
- May be required to walk on uneven terrain to get to client homes
- The employee must occasionally lift and /or move up to 25 pounds
- Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus
