Job Summary
Health Plan Behavioral Care Manager (Onsite) assumes responsibility for utilization and case management of behavioral health cases; collaborates with care management nurses for co-management of more complex cases; contributes to the Population Health Plan Initiative; requires an Associate's/Technical Degree (Bachelor's/Master's preferred) and active state-licensed clinical credentials (LCSW or Psychologist) to practice, with on-site duties in the Sacramento metropolitan area; duties include developing care plans, reviewing medical records, serving as liaison to members and providers, and developing programs for population health initiatives.
Required Qualifications
- Associate's/Technical Degree or equivalent required
- Licensed Clinical Social Worker (LCSW) or Licensed Psychologist required
- Licensed Clinical Social Worker (LCSW) in state of practice required
