Job Summary

The Health Navigator plays a crucial role in ensuring quality patient and family-centered care by managing referrals and providing direct patient services. Responsibilities include acting as a contact for patients and healthcare providers, processing referral documents, managing time-sensitive referrals, and interfacing with insurance companies. The position involves identifying social determinants of health (SDOH) issues and connecting patients with community resources to improve health outcomes. Key skills required include effective communication, strong organizational abilities, and proficiency in electronic medical records (EMR). A minimum of an associate’s degree in social work or related field is required along with relevant experience and BLS certification.

Required Qualifications

  • Associate’s degree in social work, human services, or a related field; or equivalent combination of relevant education and experience.
  • Experience working with people experiencing homelessness, immigrant, and traditionally underserved populations.
  • 2+ years in a clinical setting or similar experience.
  • BLS certification
  • Bilingual: English/Spanish, English/Hmong highly preferred.
