Job Summary
Connect neighbors to primary care, behavioral health, dental, and specialty resources while building trusted relationships to navigate complex healthcare systems. Coordinate referrals with warm handoffs to providers and partners, deliver health education and nutrition programming, and conduct home visits to assess changing needs. Maintain relationships with clinics, FQHCs, and hospitals, then track all referrals and outcomes in Salesforce to ensure integration into whole-household success plans. This role primarily works 8:00 AM–5:00 PM, Monday through Friday, with occasional evenings and weekends based on program needs.
Required Qualifications
- Bachelor's degree in Public Health, Health Education, Health Promotion, Nursing, Medical Social Work, Substance Abuse/Behavioral Disorders, or related field
- 3–5 years of experience in the healthcare, community health, or social service field
- Bilingual: English/Spanish required
- Proficiency with Salesforce or comparable client tracking system

Desired Qualifications
- Community Health Worker (CHW) Certification
- Experience in Medical Social Work
- Familiarity with SDOH screening frameworks or comparable whole-household assessment tools
- Experience working with uninsured or underinsured populations and navigating low-cost or free care options
