Job Summary
Health Navigator role inside partner clinics to support doctors with patient-facing, administrative, and logistical tasks, enabling high-quality care in a value-based model. Responsibilities include developing trusted relationships with patients and providers, data entry into the Main Street software, calling patients to schedule visits or remind them to pick up medication, ordering and scheduling procedures and screenings, identifying and tracking gaps in patient care, maintaining patient interactions in the EHR, educating patients about healthcare options and insurance benefits, helping patients access community resources, coordinating follow-up care after hospital discharge, and collaborating with clinic staff with a compassionate, detail-oriented, and proactive approach. Requires full-time commitment (40 hours/week), a GED or high school diploma, and an active unencumbered driver’s license. Expertise in using multiple software platforms, strong communication and problem-solving skills, and a service-oriented mindset are valued.
