Job Summary
Develop strong relationships with patients by meeting them during check-in processes, calling to schedule visits or medication pickups, and educating them on healthcare options and insurance benefits. Enter data into the Main Street software platform for reporting and tracking, order and schedule procedures and screenings, and identify gaps in patient care. Maintain records of interactions, communicate with providers using electronic health records, and help coordinate follow-up care after hospital discharge. Assist with administrative and logistical tasks to support doctors in delivering high-quality care, while helping patients access community resources. Requires an active unencumbered driver's license and familiarity with multiple software platforms.
