Job Summary
Develop strong relationships with patients by meeting them during check-in, calling to schedule visits or medication pickups, and educating them on healthcare options and insurance benefits. Enter data into the Main Street software platform for reporting, order and schedule procedures and screenings, and track progress on patient care gaps. Maintain records of interactions, communicate with providers via electronic health records, and help coordinate follow-up care after hospital discharge. Assist with administrative and logistical tasks, including contacting pharmacies to refill prescriptions and identifying community resources. Work full-time Monday through Friday from 8am to 5pm with an active unencumbered driver's license required.
