Job Summary
Provide support to participants and members in case and disease management programs to meet treatment goals in coordination with case managers. Assist patients in navigating clinical and community resources, connecting them to health screening programs, and facilitating access to medical, social, and behavioral services. Document care coordination, care gaps, and discharge planning needs in care management systems independently. Establish ongoing relationships by facilitating communication with participants, caregivers, and providers to coordinate care across settings. Participate in case conferences and interdisciplinary meetings with the CM team and medical directors to support effective care coordination. Educate members about benefits, services, eligibility, and referrals while promoting satisfaction and retention.
Required Qualifications
- High school diploma or equivalent education
- Two (2) years in an acute care facility
- Experience in case management
- Experience in emergency department
- Knowledgeable of managed care, including contracted and non-contracted providers
- Spoken and written fluency in English
- Ability to maintain confidentiality of all PHI in compliance with state and federal law
- Ability to maintain confidentiality of all PHI in compliance with BTP policy
- Must pass a background check/drug screen
