Job Summary
Health Navigator role focused on evaluating, supporting, and coordinating healthcare access through community resources. Responsibilities include comprehensive care coordination across episodes of care, health coaching, conducting assessments (in collaboration with the interdisciplinary team), coordinating follow-up and visits (including potential virtual assessments), assisting patients with accessing community resources and services, and education on health-related topics. The position involves working with patients to develop and monitor care plans, documenting progress, identifying barriers to care, and communicating with care teams and community partners. Requires fostering patient education, adherence, telehealth/clinic workflow support, and adherence to confidentiality and policy guidelines; work may involve in-person visits and occasional travel within a multi-county region. Qualifications include a Bachelor’s degree in a related field (with preferred LCSW), at least one year of experience in outreach and care coordination, and certification as a Community Health Worker, plus a valid driver’s license.
