Health Navigator (60099)

Engage chronically unhoused adults who are frequent users of hospital and emergency services annually. Conduct patient screenings to evaluate social, mental health, and medical needs, creating individualized action plans to transition patients from emergency care to preventative primary care. Build trusting relationships with patients and collaborate with healthcare providers, social workers, and case managers to connect them to appropriate resource services, including housing, benefits, and medical respite centers. Coordinate referrals, assist patients in completing applications for programs addressing housing instability and substance use, and help establish primary care providers and post-discharge appointments. Document assessments, track outcomes, and report data to leadership and hospital partners. Participate in multi-disciplinary team meetings to coordinate patient services. Requires a high school diploma or GED, two years of experience as a medical assistant or case manager, direct patient care experience, and a valid Arizona driver's license. Bilingual-Spanish is desired.
