Job Summary
Health Monitor/Health Aide responsible for monthly visits to affiliated provider homes to ensure proper integration of OCFS/DOHMH regulations and CACFP monitoring, CACFP enrollment duties, and regular in-person coaching with corrective actions. Supports health CAPs with Education Specialist, documents visits, stays current on NY State license/registration requirements, communicates with providers and families, collaborates on resource referrals, assists with workshops and network activities, attends CACFP trainings and monitors Kid Kare accounts as needed, and participates in staff meetings. Requires an Associate Degree or CDA credential and nutrition, health, and safety training; travel within the FCC Network catchment area; bilingual English/Spanish preferred; full-time schedule (35 hours/week).
