Job Summary
Health Manager responsible for supporting members in achieving self-efficacy in managing health conditions through a collaborative process of assessment, planning, implementation, coordination, monitoring, and evaluation of self-management strategies and care services. Roles include member-centered planning, education and interventions for condition management, coordination with providers and teams, handling a high volume of member calls, interpretation of coverage and program information, and maintaining records to meet accreditation and quality standards. Requires an active nursing license (RN) or RD/RDN or Respiratory Therapist credential, an associate-degree-level education, and the ability to work remotely within Arizona with optional onsite requirements as needed for business considerations.
Required Qualifications
- 2 years of experience in full-time equivalent direct clinical care to the consumer
- Associate’s Degree in general field of study or Post High School Nursing Diploma
- Active, current, and unrestricted nursing license in AZ or another NLC-recognized state (RN) OR active RD/RDN registration OR active Respiratory Therapist/RC Practitioner registration
Desired Qualifications
- Active RN license (NLC-recognized) or RD/RDN license or Respiratory Therapist license
- 2 years of direct clinical care experience
- Associate’s Degree in general field of study or Post High School Nursing Diploma
- Diabetes Care and Education or related certifications are not required but preferred (if mentioned)
