Health Liaison

Manage a caseload of clients by conducting primary care screenings, monitoring health status, and proactively identifying potential risks. Obtain screenings from Primary Care Physicians, scan results into the Electronic Medical Record, and provide ongoing health education based on identified conditions. Make and track referrals to primary care and specialty services, assist with appointment scheduling, and monitor client attendance at external visits. Collaborate with FCC providers to develop comprehensive care plans and coordinate care with health providers with client consent. Document all activities while ensuring the program adheres to CARF and DMH certification standards and agency policies.
