Job Summary
Assess patients' needs to connect them with vital resources such as health insurance, basic food, and transportation. Recertify clients and complete applications for Medicaid, Apple Health, Qualified Health Program, and other state or federal assistance programs while identifying eligibility and providing education. Maintain confidentiality by documenting interactions in approved programs and meeting clients in private spaces. Build relationships with external organizations to advocate for clients and troubleshoot access issues. Provide consistent documentation and patient follow-through to complete application processes and ensure correct insurance assignment. Review and build reports to ensure patients receive eligible education on available services. Participate in Annual and Open Enrollment periods by sharing updates with sites and acting as a point of contact for questions. Represent HealthPoint at community events to provide resources to clients and attend required internal and external meetings.
Required Qualifications
- Technical College or Associate's Degree
- 1 to 2 years of experience with Medicaid medical insurance
- Ability to read and interpret technical and other complex documents
- Ability to write routine correspondence such as letters and memos
- Ability to present information in one-on-one and small group situations to internal and/or external clients
- The ability to define problems, collect data, establish facts, and draw valid conclusions
- Ability to interpret an extensive variety of technical instruction
- Constructive thinking and alternative short-to long term solutions
- Action plans must be developed and implemented
- WHPF Certified Navigator within one (1) month of being hired
- Proof of vaccination for COVID-19
- Proof of immunizations and/or immunity to MMR (measles, mumps, rubella)
- Proof of immunizations and/or immunity to Varicella
- Proof of annual Influenza immunization
- Proof of TB QuantiFERON Gold Titer
- Tdap (within last 10 years)
- Hepatitis B
- Adherence to exchange and navigator license requirements including recertifications, updates and annual in-service days
Desired Qualifications
- Preferred knowledge of computer equipment
- An intermediate level of Word, Excel & Outlook
- Technical College or Associate's Degree and one (1) to two (2) years of experience with Medicaid medical insurance preferred
