Job Summary
Health Hub Care Coordinator responsible for patient intake, record keeping, and coordination of patient care with external stakeholders. Core duties include completing intake documents, scheduling and managing walk-in appointments and follow-up, supporting Health Hub Nurse Practitioner and LCSW with record keeping, arranging transportation, coordinating with health insurance navigators for uninsured patients, and participating in interdisciplinary team meetings with internal stakeholders. Also expected to manage office/patient flow and assist with outreach events as directed.
Required Qualifications
- 1-3 years of care/care management experience
- Driver's License
