Job Summary
Conduct enrollment activities by providing families with goals and obtaining consents, demographic information, and necessary documents. Perform monthly required contacts and complete The Child and Adolescent Needs and Strengths (CANS-NY) assessment every six months for children and youth on your caseload. Conduct health assessments including developmental screens and HIV/Sexual and reproductive health screenings as required by regulatory bodies. Coordinate with multidisciplinary teams, caregivers, and community providers to deliver primary and specialized care services while monitoring client conditions that necessitate treatment changes. Ensure team members are aware of Emergency Room, inpatient hospitalization, and residential care admissions and discharges, and complete Mental Health Coordination Unit forms within prescribed time frames. Update health records in Connections and finalize billing documents for the Health Home/Managed Care Organization. This role supports the Children's Health Home Care Coordination team, focusing on family-centered care continuity across education, foster care, juvenile justice, and health systems.
Required Qualifications
- Bachelor's degree in health or human services, or another related field
- Must have a New York State ID
- Must demonstrate excellent communication, time management and organizational skills
