Job Summary

Conduct outreach, assessment, and service planning for clients with severe mental illness, substance use disorders, or chronic conditions, including children with severe medical, mental health, and substance use diagnoses. Perform home visits and community work at hospitals and emergency rooms to gather information for intake, reassessments, and care plans. Coordinate medical, mental health, and substance use services while monitoring client progress through telephonic and direct contact. Transport clients to appointments, assist in crisis intervention, and ensure follow-up on prescriptions and discharge aftercare. Participate in case conferences with health home team members and resolve barriers to effective service utilization. Maintain client contact records in databases and complete required CANS training and certification.

Required Qualifications

  • Master's Degree in social work or related field plus one year of experience
  • Bachelor's Degree in social work or related field plus two years of experience
  • CASAC or High School Diploma or educational equivalent and significant combination of education and experience in criminal justice or working with substance using and/or mentally ill population
  • Experience providing medical, mental health or substance use focused care coordination services to individuals with Serious Mental Illness, developmental disabilities, substance use disorders
  • Experience working with children
  • Experience in criminal justice
  • Knowledge of health care environments, clinical terminology and health information systems
  • Valid NYS (New York State) Driver's License
  • Willingness to transport clients to appointments using own vehicle
  • Proof of valid NYS car insurance
  • Defensive Driving Certificate
  • Valid registration, inspection and Automobile Insurance Liability of a recommended minimum of 100,000/300,000
  • Excellent organizational and time management skills
  • Interpersonal skills
  • Verbal and written communication skills
  • Proficiency in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software
